Refund Policy
1. Purpose
This Refund Policy outlines the terms and conditions under which Prominent Toxicology Screenings (“the Company”) will issue refunds for services provided in connection with toxicology testing, analysis, and reporting. Our goal is to ensure transparency and fairness while maintaining the integrity of laboratory operations and compliance with applicable laws and regulations.
2. Non-Refundable Services
Due to the nature of toxicology testing and laboratory analysis, all testing services are generally non-refundable once a specimen has been collected or analysis has begun. This includes, but is not limited to:

Urine, blood, saliva, or other specimen testing

Chain-of-custody documentation and handling

Laboratory processing or reporting

Administrative or courier fees
Once testing begins, consumable materials and resources are used and cannot be recovered.
3. Refund Eligibility
Refunds may be considered only under the following circumstances:

Duplicate payment: A refund will be issued if a client is accidentally charged more than once for the same service.

Service cancellation prior to specimen collection: If a test is canceled before sample collection or processing, a full or partial refund may be granted.

Test not performed: If the Company is unable to perform the test due to equipment failure, specimen rejection (not due to client fault), or laboratory error, the client may receive a full refund or retest at no cost.
4. Refund Process
To request a refund, the client must:

Submit a written request to Kejone.Denson@ProminentTox.org within 14 days of the transaction date.

Include proof of payment, test ID (if applicable), and reason for the refund request.

The Company will review all requests within 10 business days and notify the client of the outcome.

Approved refunds will be processed using the original payment method within 5–10 business days of approval.
5. Non-Refundable Conditions
No refunds will be issued if:

The specimen is collected and testing has begun.

The test results are delayed due to factors outside of our control (e.g., shipping delays, incomplete paperwork, or client error).

The client disputes or disagrees with test results or outcomes.

Services were purchased through a third-party provider; such refunds must be handled directly with that provider.
6. Dispute Resolution
Any disputes regarding refunds or payments shall be addressed in accordance with the governing laws of Oklahoma, and the client agrees to attempt informal resolution before pursuing formal legal action.
7. Policy Updates
Prominent Toxicology Screenings reserves the right to modify this Refund Policy at any time. Updates will be posted on our website and become effective immediately upon posting.
